How To Register With Us
To register with the Practice please visit your nearest surgery and complete a ‘Registration Form’ and a ‘New Patient Health Questionnaire’. You can alternatively download this form, and print it at home.
You will be registered with the Practice and can choose to consult any of the doctors at any of the surgeries. Wherever possible it is preferable to see the same doctor each time as this improves continuity of care.
